CORI is an acronym for Criminal Offender Record Information. It is a background check to see if there is record of any criminal offense against you. As required by the Archdiocese of Boston, any person 17 years of age or older who volunteers in any capacity within the Archdiocese that puts them in any possible contact with children or the elderly, needs to be CORI checked on an annual basis. In addition to some basic information, you will be required to provide the last 6 digits of your social security number, as well as your photo ID at time of signing. It takes less than 10 minutes to complete.
Below you will find a link to the 2016 CORI Form. Please download, print, and complete all sections EXCEPT for your signature. You are required to provide your signature IN PERSON, as well as your photo ID, as mentioned above. You can drop off your form to Fran at any time that Faith Formation Classes are occurring, or you can drop off to a staff member at the parish office during normal business hours:
Monday, Tuesday, Thursday… 9:00 am—5:00 pm
Wednesday… 9:30 am—5:00 pm
Friday… 9:00 am—3:00 pm
