For over ten years now, it has been required that all parish volunteers, with even a remote possibility of access to children and other vulnerable parishioners, annually undergo a CORI (Criminal Offender Record Information) background check. Without your full cooperation in adhering to these guidelines, we would be hard pressed to be able to continue the ministry offerings that many have become so dependent on, and at a level many have come to not only expect, but appreciate. There are some important and necessary documents listed below that we need to process in order for St. John the Evangelist Parish to remain “in compliance”, as required by the guidelines set forth by the Archdiocese of Boston.

The CORI Form requires that you provide the last 6 digits of your social security number and present us with your photo ID. It will take less than 10 minutes to complete.
NOTE: This form needs to be renewed every year.

Sexual Harassment Policy: This document must be distributed annually to all staff members and volunteers.

Code of Conduct & Conflict of Interest Policy  
This document also requires annual distribution. A witness-signed ‘read receipt acknowledgement’ page must be turned in and kept on file in the parish office.

Code of Ministerial Behavior   
If you have already signed this document last year, you need not sign again this year. If and when changes are made to the document by the Archdiocese, you will be directed to sign again. If you have never signed it, a witness-signed ‘read receipt acknowledgement’ page must be turned in and kept on file in the parish office.

All these documents can be downloaded. IF YOU CHOOSE TO DOWNLOAD THE FORMS, PLEASE DO NOT SIGN AS YOUR SIGNATURE REQUIRES THE PRESENCE OF A PARISH STAFF MEMBER. Although at first glance it seems like an insurmountable amount of reading, we are confident that once you begin reviewing these documents, you will quickly find that the information provided is of a critical nature. It has been proven that our familiarity with them makes it easier for caring adults, like yourself, to communicate any concerns to the appropriate person(s) regarding a warning sign/red flag of inappropriate behavior. DON’T FORGET TO BRING YOUR PHOTO ID!
